this is a required NASA form all drivers must fill out. NASA TX will likely give these w/ the driver packet, but go ahead and print and fill it out now so you will have it ready once you arrive. give it to one of the directors.
i have also attached the Race Car Tech sheet. this one gets filled out and turned in to race tech at the drivers meeting sat morning. you will not get your track access sticker untill you turn this in filled out and signed. put it in your log book - thats wherei keep mine.
